POSITION DESCRIPTION

The Case Manager provides case management for clients who may be diagnosed with chemical dependency or co-occurring chemical dependency and mental health disorders. The Case Manager also functions as an advocate and support person to individuals assigned to their program and links them to appropriate services and monitors client progress. 

JOB DUTIES AND RESPONSIBILITIES

  • Advocates in the client's best interest.
  • Conduct client intake, screening, assessment, orientation, and related reporting. 
  • Develop and implement case management plans for clients that address goals of obtaining stable housing, gainful employment, engagement in behavioral health recovery support, and linkage to ancillary services to support those goals.
  • Develop transition and discharge plans with clients that include provisions for behavioral health disorders.
  • Coach clients on engagement with treatment plans and self-help recovery services, individualized health and wellness goals, and self-responsibility. 
  • Facilitate life skills group with fidelity to evidence based practices.
  • Prepare and maintain clinical records in accordance with policy and procedure. 
  • Interface positively with community partners.
  • Participate in client case reviews and team meetings.
  • Make referrals to appropriate resource agencies and document outcomes of referrals.
  • Provide crisis intervention, when needed.
  • Maintain client rights as defined by DCF.
  • Maintain the highest standards in ethical conduct and confidentiality.
  • Maintain clinical records in accordance with Agency standards.

REQUIRED KEY PERFORMANCE INDICATORS

  • Complete targeted case management direct service hours 1352 hours per year.
  • Concurrent Documentation is expectation and all notes completed same day of service.
  • Meet Recovery goals through change of level of care within set time frame 90% of the time.
  • Will have 99% accuracy in  E.H.R. on entering information.
  • 100% of events provided will be on the active treatment plan.
  • 100% of required trainings will be completed within specified time frames.
  • No authorizations no service.
  • No show rate below 10%.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

  • Bachelor's degree with major course work in human services field.
  • Minimum of 5 years relevant experience working with adults with substance abuse/mental health issues.
  • Must have a clear understanding of the characteristics and problems associated with mental health diagnosis(es) or co-occurring mental health and substance abuse diagnosis.
  • Knowledge of substance use disorders and the principles of recovery.
  • Knowledge of Trauma Informed Care helpful.
  • Strong crisis intervention skills required.
  • Must have CPR Certification and first aid.
  • Good written and verbal communication skills.
  • Effective time management and organizational skills.
  • Computer literate.
  • Ability to work in a team setting.
  • Ability to act as a positive role model.
PHYSICAL DEMANDS

REGULARLY REQUIRED:  to sit, use arms/hands to reach, manipulate, handle, or feel objects, tools, or controls; written and verbal communication, ability to hear.

OCCASIONALLY REQUIRED: to stand, walk, stoop, kneel or crouch and lift and/or move up to 10 pounds. 

WORK ENVIRONMENT

The work environment is that of a substance use treatment milieu which may include:

Exposure to clients infected with Hepatitis B, HIV, TB, or other Infectious Diseases

Flexibility to work days, nights, weekends, and holidays.
